HERE is the link to the September District Report Form!  The deadline for these monthly District Report Forms will always be due on the 10th of the month, so you have until October 10th to fill the one for September out! These monthly forms allow the Indiana District to stay connected with clubs and assess the best way to serve them! These forms should typically be filled out by the club secretary, but any other club officer and/or the club advisor can fill them out as well. However, it is strongly encouraged that the club officers and club advisor fill the form out altogether!
