HERE is the link to the November District Report Form! The deadline for these monthly District Report Forms will always be due on the 10th of the following month, so, for example, you have until December 10th to fill out the one for November! These monthly forms enable the Indiana District to stay connected with clubs and assess the most effective way to serve them. These forms are typically filled out by the club secretary, but any other club officer and/or the club advisor can fill them out as well. However, it is strongly encouraged that the club officers and club advisor fill out the form altogether!
